Conclusive Coordination is looking for someone to join our small team of Support Coordinators!

Who We're Looking For: We are open to candidates with minimal or no experience in support coordination, as long as you possess the motivation and desire to learn. We value initiative and a positive attitude.

Role Overview: As a Support Coordinator, you will initially manage a small caseload of approximately 10 clients, with the opportunity to expand as you gain confidence and experience in the role. The position involves approximately 5-10 hours of administrative work per week to ensure a smooth transition into your responsibilities.

Key Responsibilities:

Administrative Tasks:

  • Completing service agreements and consent forms.
  • Attending meetings and writing meeting minutes to document discussions and decisions.
  • Conducting research to assist coordinators in finding local allied health and support providers for our clients.
  • Sending welcome emails to new clients to ensure they feel supported from the start.
  • Managing the admin email for inquiries and correspondence.
  • Being available during business hours incase we need to divert our phones.
  • Updating client support coordination plans to ensure effective support.
  • Managing and inputting client data into our database for accurate records.

Support Coordination Tasks:

  • Assist NDIS participants and their families to meet goals and optimize their NDIS plans.
  • Build strong relationships with service providers and allied healthcare professionals.
  • Foster connections with participants and their families, focusing on capacity building and enabling choice and control.
  • Provide plan review reports to the NDIA that clearly articulate participant needs.
  • Develop sound plan implementation practices for the duration of the NDIS plan.
  • Deliver transparent and effective services to participants.
  • Empower participants to understand their plans and implementation.
  • Collaborate effectively with internal and external teams to communicate participant needs.
  • Innovate within organizational processes, adhering to best practices and NDIS guidelines.

Job Type: Subcontract

Pay: $50.00 – $60.00 per hour

Expected hours: 5 – 20 per week
