Position Summary:
Reports to the Operations Manager and will work under the supervision of a Registered Healthcare Professional (RN or RPN), in a Team Based Care Model.
As a member of the health care team, the Personal Support Worker (PSW) provides direct quality personal care to patients in accordance with their respective plan of care.
The PSW provides for the individual needs of assigned patients by facilitating, promoting and participating in the maintenance of the patient’s optimum level of physical, emotional, social, cultural, intellectual and spiritual function in accordance with their plan of care.
Qualifications:
Works collaboratively with other team members; interacts tactfully and courteously with patients, patients’ families, visitors, and all members of the health care team.
Performs personal care and selected nursing interventions to promote comfort and safety for the patient in accordance with established standards of patient care. This includes:
Assisting with grooming activities and dressing including showers, baths and skin care needs as directed
Assisting in serving meals and nourishments, feeds patients and reports/records patient’s intake accurately.
Providing for and assist with patient’s elimination needs as determined in the individual care plan.
Assisting patients with transfers and ambulation through the use of walkers, canes, wheelchairs, and other supportive devices as noted in the care plan.
Completing vital signs for patient as appropriate
Observing and reporting changes in patient’s physical and emotional condition to the healthcare team
Completes documentation within scope indicating patient’s condition, care and specific treatments.
Assist nursing staff in answering call bells
Promotes the well-being of the patient by participating in the overall activation process. This includes the awareness of the whole person considering spiritual, cultural, and psychosocial needs.
Maintains a safe, neat, and clean environment for patients and staff. Recognizes and reports any safety hazards and follows all safety policies and procedures.
Additionally the PSW is required to possess a skill set of abilities that will enable them to perform their job effectively. This includes experience in providing general care and support, competency in working under pressure and proficient in oral and written communication.

Education and/or Certification:
Successful completion of Personal Support Worker diploma through an accredited Community College/Institution - Required
Current certification in Standard First Aid and CPR – Required
Certification in Gentle Persuasive Approaches In Patients with Dementia or P.I.E.C.E.S or equivalent – preferred
