We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a crucial role in service definition, planning and delivery to poeple with disabilites, who live in a community setting. Proficient command and comprehension of the English language; in both reading and writing, effective interpersonal skills, use of information systems technology for case management documentation and report writing, knowledge of CET, and Accommodation Standards and Licensing is an asset. Responsibilities:

- Provide case management services to clients, including assessment, planning, and coordination of care

- Conduct client interviews to gather necessary information and assess needs

- Develop individualized care plans based on client goals and objectives

- Coordinate with healthcare providers and community resources to ensure clients receive appropriate services

- Monitor client progress and make adjustments to care plans as needed

- Maintain accurate and up-to-date client records using computerized systems

- Assist with administrative tasks such as filing, data entry, and record keeping

- Provide excellent customer support by addressing client inquiries and concerns in a timely manner

- Demonstrate professionalism and phone etiquette when communicating with clients and stakeholders

Requirements:

- Bachelor's degree in social work or related field preferred

- Previous experience in a the disability services field, as a medical office or in an administrative role is highly desirable

- Strong organizational skills with the ability to prioritize tasks effectively

- Excellent communication skills, both written and verbal

- Proficient in computerized systems for data entry and record keeping

- Ability to provide exceptional customer service and support

- Knowledge of case management principles and practices is a plus

Note: All positions at our organization are paid positions, including internships.

Job Types: Full-time, Permanent

Salary: $24.15-$34.38 per hour

Expected hours: 40 – 48 per week
