Intake RN

The Intake Nurse plays a critical role in the patient admission process, ensuring that all incoming referrals are thoroughly reviewed for both clinical and program appropriateness. As a key liaison between the agency and referral sources, the Intake Nurse maintains open lines of communication to facilitate accurate and efficient patient intake. Supported by the Intake Coordinator, this role demands strong clinical judgment, attention to detail, and excellent communication skills to uphold the agency's mission of delivering high-quality care at home. This is an office-based position. While prior homecare experience is appreciated, it is not required.

Education and/or Experience:

• Graduate of an accredited nursing program
• 2-years recent experience as an RN in community health, medical/surgical or in an acute or skilled long term care setting preferred but not required (willing to train the right candidate)
• Possess a valid RN License issued by the State of New Hampshire
• Ability to read and interpret written documents
• Computer competency including the ability to retrieve information necessary to complete job responsibilities

Licenses/Insurance:

Access to operable, insured motor vehicle and valid driver's license with acceptable record of safe driving practices.

ESSENTIAL FUNCTIONS:

• Responds to and/or triages calls in a timely manner and documents all telephone contacts, patient care observations and interventions.

• Receives and records accurate and thorough referral information required for services.

• Monitors all portals and platforms used for referral management for incoming referrals and orders.

• Works with referral sources to process urgent referrals and schedule them as soon as needed to meet patient/referral source needs.

• Creates charts accurately and timely. Inputs all information without error, including demographics, insurance, medications, intake note, Face to Face information etc.

• Records the outcome of all inquiries and referrals received and provides appropriate follow-up when an inquiry or initial referral cannot be serviced during office hours

• Communicates with the interdisciplinary team to ensure proper patient follow-up for complex patients or patients who may not be identified as being referred to the appropriate program

• Provides accurate information to customers making telephone inquiries regarding Home Care and, Hospice and other agency services.

• Assists with document management which includes but not limited to sending documents to Providers via the EMR and/or fax, receiving orders and uploading to the patient's chart as appropriate.

• Performs audits as necessary under the direction of the Director of Clinical Services

• Assists with EMR training and implementation.

• Demonstrates good organizational skills and exceptional customer service skills

• Uses equipment and supplies in an appropriate and cost-effective manner

• Acknowledges that all information related to employees, clients and their families, and NCHHHA business practices is confidential

• Practices proper body mechanics and observes safety measures

• Adheres to all safety policies including reporting/ removing hazards immediately

• Demonstrates knowledge and compliance with infection control policies

• Follows instructions, responds to management direction

• Works cooperatively with other NCHHHA personnel

• Complies with NCHHHA's Standard of Conduct

• Required completion of time sheets and other records in accordance with agency policy and State and Federal regulations
