RN Case Manager Home Care

We are seeking a compassionate Registered Nurse (RN) Case Manager to join our interdisciplinary hospice team, providing expert care to seriously ill patients. This full-time position offers a $5,000 sign-on bonus and no on-call shifts, with a Monday-Friday schedule from 8:00 a.m. to 5:00 p.m.

Key Responsibilities

• Manage a reasonable caseload with up to 5 patient visits per day.
• Provide comfort, care, support, and education to patients and families.
• Participate in weekly interdisciplinary meetings and collaborate with a dedicated support team.
  
  

Requirements

• Valid RN license in Missouri and Kansas.
• Minimum one year of bedside experience in hospice care preferred.
• Strong communication and organizational skills.
