JOB SUMMARY: The Resident Care Coordinator (RCC) is responsible for the coordination of resident care support and staffing necessary to meet the needs of all residents in collaboration with the Health Services Director, Memory Care Coordinator and Wellness Nurse. The RCC provides communication with physicians and other medical care professionals and families related to resident care and services. The RCC is responsible for care staff schedules, responding to staff call-offs and ensuring the daily staffing schedule is complete based upon occupancy and resident needs.

ESSENTIAL JOB FUNCTIONS:

  • Maintains confidentiality regarding information about residents and their families. Strives to preserve resident privacy. 
  • Maintains communication with health care professionals at community in-patient facilities such as acute care, psychiatric, or convalescent hospitals when a Maple Tree Village resident is under their care. 
  • Coordinates prospective resident assessments for the clinical leadership team.
  • Monitors and audits completion of resident assessments and 602’s to ensure compliance.
  • Remains alert to the residents’ condition and report needs, changes, and concerns to the RCFE Administrator, family, physicians and other community health professionals, as appropriate. 
  • Coordinates the scheduling of resident care conferences with all required participants.
  • Assists new residents with arranging personal items and help familiarize residents and their family members with their new surroundings
  • Arranges for coverage of job duties within the department during employee absences either through delegation or personal completion of duties
  • Monitors the ordering of medications and medical supplies as requested. 
  • Maintains inventory of health-related supplies at all times. 
  • Assists when requested with orientation and training of staff. 
  • Develops a schedule to promote regular orientation and in-service education, and one-on-one staff development as needed. 
  • Prepares care team new hire orientation plan in collaboration with Health Services Director.
  • Assist with coordinating annual staff training in first aid and/or CPR in conjunction with AL RCM and MC RCM.
  • Assists with direct resident care as needed. 
  • Assists AL RCM in coordinating incidental medical, dental, vision, hearing and podiatry care for residents.
  • Regularly reviews employee time records to ensure that employees adhere to scheduled shift times, meal breaks, rest periods and are accountable for accurate time clock punches. Identify early, late and missed punches and ensure meal periods are taken. Document disciplinary action, as necessary.
  • Provides care staff with copies of the most current resident service plans and staff tasks and monitor to ensure services are provided and documented according to company policy
  • May work with other staff to plan and conduct special events fundraisers and ACC programs. 
  • Attends required in-service and training programs. 
  • Participates in the change of shift meetings to communicate pertinent information regarding resident care needs.
  • Oversees central storage and delivery of medications.
  • Demonstrates knowledge of infection control, i.e., adequate hand-washing, blood-borne pathogen procedures, universal precautions, etc.
  • Performs other duties consistent with the position as assigned.

QUALIFICATIONS/MINIMUM REQUIREMENTS:

  • Knowledge of the requirements for providing care and supervision of residents in a Residential Care Facility for the Elderly (RCFE) licensed community.
  • Ability to maintain and comply with the regulations related to the maintenance of the residents’ medical records.
  • Two (2) years’ experience working in a Residential Care Facility for the Elderly (RCFE) providing direct care and/or medication assistance.
  • Successful completion of the ACCs medication training program within 60 days of hire.
  • Obtain and maintain a valid Food Handler’s certification within 60 days of hire.
  • Obtain and maintain valid first aid and CPR certification within 30 days of hire.
  • Able to maintain a calm and professional demeanor in stressful situations.
  • Ability to work in a multi-ethnic and multicultural environment.
  • Effective communication skills, both verbal and written. Fluency in additional languages is preferred.
  • Flexibility and willingness to work evenings or weekends as needed.
  • Knowledge and use of proper body mechanics and safety mechanics. Compliance with all OSHA (Occupational Safety and Health Act) regulations is required.
  • Ability to maintain a pleasant, helpful and courteous manner with all residents, personnel, volunteers, and visitors.
  • Basic understanding and proficiency in Microsoft Office programs (Word, Excel, etc.) and electronic health records systems (Point Click Care).
