What are the responsibilities and job description for the Program Nurse position at Active Generations?
As the Program Nurse, you will assist the ADS Program Manager in providing professional and direct nursing services. This role assists in curating care plans, dispensing medications, providing health care education to participants and their caregivers, and consulting with physicians, social workers, or any other health care professional who is directly involved in the participant’s care and treatment. This position helps with toileting, showers, leading groups, and supporting the program staff working on the floor.
What You’ll Do:
- Coordinate therapy services and flu vaccinations for participants and Adult Day Services staff.
- Conduct nursing assessments during intake and update annually.
- Develop and maintain individual care plans, record documentation, and charts.
- Follow up with physicians after care conferences as needed and address medical concerns with family caregivers or physicians.
- Document monthly progress notes for VA participants and assist with VA chart reviews onsite at Day Break.
- Order medical supplies and check Active Generations' three AEDs on the first of each month.
- Coordinate nursing student projects and facilitate participant use of the NuStep machine.
- Educate staff on Universal Precautions and ensure adherence.
- Assist participants with meal setup, facilitate groups as needed, and provide program tours.
- Attend workshops, seminars, and continuing education to stay updated on developments in the field.
What We’re Looking For:
- Registered Nurse (RN) with 1-2 years of experience in geriatric nursing or Licensed Practical Nurse (LPN) with 3-5 years of geriatric nursing experience.
- Valid South Dakota Nursing License.
Physical Requirements:
- Regularly lift and transfer participants, with frequent movement including stooping, kneeling, crouching, and reaching.
- Ability to lift/move up to 50 pounds regularly and up to 100 pounds occasionally.
- Stand, walk, and move for 3-4 hours without the need to sit.
- Use of vision skills including close, distance, peripheral, depth perception, and focus adjustment.
- Perform personal care and health-related tasks within the scope of care.
- Sit occasionally and view computer screens for short periods (about 50% of the time).
- Proficient in reading, writing, speaking, and applying mathematical concepts.
- Competent in using Microsoft Office (Word, Excel, Outlook) and basic computer operations in a Windows environment.
- Strong problem-solving, critical thinking, and multitasking skills.
