Summary/Objective

Under the clinical oversight of the GUIDE Medical Director, and the administrative supervision of GUIDE Manager, the Nurse Practitioner (NP) provides clinical leadership and to the interdisciplinary team (IDT) and supervision of the GUIDE clinical and support staff. NP provides direct medical care as needed to participants at the GUIDE clinic, at home, via telehealth or in other healthcare settings. NP serves as liaison between the program and community physicians and healthcare entities.

Essential Duties/Responsibilities

Administrative

· Work cooperatively with the GUIDE Manager to ensure quality care delivery to all participants.

· Supervise clinical and support staff.

· Lead specific projects as appropriate and directed by GUIDE Manager or Medical Director.

· Establish a network of appropriate consultative physicians who can assist in the care of GUIDE participants.

· Participate in 24-hour coverage as required to maintain 24-hour 7 day/week coverage for the GUIDE program

· Assist the GUIDE administrative team in the development of written guidelines and protocols as appropriate for the GUIDE program

· Oversee compliance with medical records procedures, including accurate and detailed documentation and signatures as required.

· Oversee Quality Improvement initiatives.

· Network with and educate potential referral sources and the medical and lay communities about the GUIDE program.

· Communicate and work positively and effectively between teams and departments, participants, families, caregivers and other professionals.

Clinical

· Provide direct supervision of clinical and support staff to ensure quality clinical outcomes

· Provide clinical expertise to assist with decisions regarding diagnosis and medical management of participants’ needs, assuring that the highest quality of medical care is provided, and that continuity and comprehensiveness of care is maintained.

· Provide medical direction and guidance to the program and other staff to assure the maintenance of quality of care for the participants and families.

· Actively participate in the IDT. Assist in developing a plan of care for each participant/family with the coordination of the physician(s) and other members of the IDT.

· Provide suggestions to the IDT that foster high-quality, cost-effective care for all participants.

· Perform comprehensive history and physicals on new referrals.

· Participate in decisions regarding appropriate admissions.

· Serve as the primary care practitioner (PCP) for participants in the GUIDE Program when requested by the participant.

· Implement care protocols for GUIDE

· Establish and implement a preventive care program as appropriate for participants enrolled in the GUIDE program.

· Manage and round on GUIDE participants admitted to long term care facilities or acute in participant facilities.

· Implement processes to capture the most appropriate diagnoses and billing codes for GUIDE participants.

· Ensure comprehensive and timely reviews of the complete admission history and physical examination for each participant, including all available hospitalizations, office notes, laboratory data, etc.

· Conduct family meetings as appropriate for GUIDE enrollees.

· Assist in improving the quality of all GUIDE documentation by the members of the IDT.

· Assist in securing medical records to better determine appropriate medical care for participants.

· Communicate and collaborate with participants’ primary care physicians and other healthcare professionals.

· Complete timely documentation in electronic medical record platform.
