Job Summary:
As a mentor you will empower the new DCW team members to make positive life choices that enable them to maximize their potential, building increased retention of our DCW staff. The Direct Care Worker Mentor position is a versatile role that combines mentoring, and direct client care job duties and responsibilities. In this position you will provide assistance with personal care services to clients with disabilities and/or chronic conditions of all ages. Assistance is limited to performance of activities of daily living (ADL and IADLs). This is a full-time, safety-sensitive position, requiring availability to cover shifts throughout the week Monday through Sunday, ranging from 2 to 12 hours each. Additionally, you will serve as a trusted guide and coach for all newly hired and existing Direct Care Workers (DCW).
Direct Care Worker Responsibilities and Duties:
- Provide direct and indirect personal care to the patient in the home setting under the direction of a client care coordinator.
- Available to provide services for ongoing clients and cover shifts. Shifts ranging from 2 – 12 hours long. Shifts can occur during the morning, day, evenings or overnight.
- Utilizes safety measure in provision of care.
- Maintains effective communication with supervisor regarding patient’s condition.
- Follows written assignment to deliver patient services developed by an appropriate health care professional. (RN, PT, ST, CCC)
- Documents and submits accurate accounts of services provided.
- Observes the patient for changes in condition and behavior and reports to supervisor.
- Maintains confidentiality of all patient and agency matters.
- Adheres to all Agency policies.
- Recognizes emergencies and has knowledge of emergency procedures.
Direct Care Worker Mentor Responsibilities and Duties:
- Serve as positive role model by modeling desirable behaviors, e.g., patience, tolerance, and reflective listening.
- Have a clear understanding of the expectations for the mentee and clearly communicate those expectations.
- Honor all commitments made with mentee.
- Encourage and support the mentee in developing milestone plans for obtaining designated goals.
- Adapt your feedback to your mentee’s learning style.
- Advise, don’t dictate, offering constructive feedback.
- Recognize your mentee’s weaknesses and build on his/her strengths.
- Be consistent and reliable.
- Attend and participate in all training sessions and meetings.
- Follow up with mentee on their successes.
- Be comfortable and able to establish appropriate boundaries with mentee.
- Recognize the role as a mandated reporter and report any problematic issues.
- Provide an evaluation of the outcomes.
- Work both independently and as part of a team with fellow peer mentors.
- The mentoring role and connection with the mentee is expected to be strictly professional.
- Maintain the highest level of care through proactive communication (weekly calls and monthly home visits) and changes to care plan as client and family’s needs change.
- Document all communication with clients, family members, direct care workers, case management and other agencies. Document and communicate all changes in the clients’ condition/schedule/ dynamics etc. with your coordinating team, case management, and/or family contact.
COMPENSATION PACKAGE:
Pay Rate: $17 - $19 Hourly
Insurance available after 60 days (medical/ dental/vision). Company pays 80% of employee monthly premium, employee pays 20%. Company sponsored life insurance coverage of $15,000 for employee. Employee Costco membership.
3 weeks Paid Time Off (begins accruing day one. Employee can utilize after 90 days of employment).

Our resources include rapid response testing for COVID-19, and we can also help you arrange for a vaccination (including transportation to and from if needed) in collaboration with AHCCCS.
