DESCRIPTION OF WORK
In the role of Housing Caseworker 3, Department of Human Services, (AAA), in the Older Adult Services Bureau/Housing Unit you will be responsible for managing a caseload of Dom Care consumers and providers. This includes initial assessment of consumers, determining level of care needed, and providing ongoing care management. You will have the opportunity to manage a caseload of SHARE (Shared Housing and Resource Exchange) Hosts and Guests. You will play a key role in assisting with the development and refinement of the housing match (SHARE) program, including the development of policies and procedures, application and intake forms, home sharing lease agreement templates, and monitoring tools and information databases.
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch
Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience
