We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers. The Nurse Practitioner / Physician Assistant provides comprehensive, family-oriented, outpatient care for clients throughout the life cycle, including health examinations and treatment of medical conditions to include follow-up on problems detected within the confines of Amoskeag Health's objectives and policies.
