We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Licensed Social Workers or Registered Nurses to join our Case Management team. Our Case Managers evaluate our member's needs and enhance their overall wellness through home visits where we do face-to-face assessments.

Starting salary is $51,899 with salary increases every year for the first 4 years.
