Aegis Health Care is currently seeking a detail-oriented Patient Care Coordinator to be part of our Atlas Medical team. Atlas Medical specializes in providing primary care for geriatric patients in Senior Living communities. This is a full-time position working Monday-Thursday 7:30am-6pm in our Anchorage office.
The Patient Care Coordinator (PCC) is an important part of the Aegis Health Care team. Aegis Health Care specializes in providing complete care for geriatric patients. The PCC provides planning, direction, and coordination of overall healthcare of residents in senior living communities. You will collaborate care between the facility, family, and providers that serve the patient.

Job Responsibilities:
- Collect personal health information and input it into the patient medical record
- Coordinate schedules of residents and providers to maximize efficiency
- Ongoing communication with the patient, patient's family, provider, and facility keeping them informed of all medical needs
- Ensure compliance with federal, state, and local regulations and standards
- Accept and perform other related duties and responsibilities as required
Skills & Qualifications
- CNA or MA license preferred
- 1-2 years medical experience preferred
- Ability to relate well to staff and other professionals
- Service oriented mindset
- Adaptive problem-solving skills
- Compassionate and caring attitude
- Valid driver's license, clean driving record, and reliable automobile
- Proof of current automobile insurance
- Criminal background check clearance/fingerprint clearance
- Negative TB test
