Role Summary:
The Hospice RN will manage a caseload of hospice patients, providing compassionate, end-of-life care in their homes. Duties include:
- Comprehensive case management, including care coordination and medication management.
- Developing individualized care plans and updating them as needed.
- Providing emotional support and education to patients and families.
- Ensuring documentation is completed accurately and promptly in McKesson/Netsmart EMR.
Requirements:
Experience:
- Hospice Case Management experience required.
- Experience providing hospice care in a home setting required.
- Proficiency with McKesson/Netsmart EMR is preferred.
Certifications:
- Active RN license in the state of Vermont or compact state license.
- BLS certification.
Job Types: Travel nursing, Contract
Pay: $75.00 - $78.00 per hour
Expected hours: 36 per week
