Job Summary:

Episcopal Church Home, our 131 bed Skilled Nursing facility has a rare opportunity for a Licensed Social Worker to join our Social Service team. This position will manage the social services and discharge planning for our 21 bed Transitional Care Unit.

Position Type/Expected Hours of Work: Full-Time, 40hrs/week working Monday - Friday

Salary: From $50,000yr , experience compensated!!!

Job Duties:

• Develops and implements a patient specific, safe and timely discharge plan for short term rehabilitation patients.
• Facilitates multi-disciplinary team meetings with patients, family, rehab, and clinical staff.
• Understands the patient specific coverage through various payment sources to coordinate care within the patient's insurance guidelines.
• Provides guidance to the patient or family on Medicare, Insurance, Medicare Advantage, and Medicaid plans for TCU and community service needs upon discharge.
• Connects patients with community resources and care needed for a successful community discharge.
• Completes necessary assessments and develops a patient specific plan of care within the Federal and State guidelines.

Minimum Qualifications:

• Licensed Social Worker
• Experience discharge planning in a skilled transitional care unit
• Experience working with a variety of SNF payers - insurance, managed care, Medicare and Medicaid.
• Knowledge of community resources
