Case Manager-NNORC
The Case Manager is responsible for engaging, planning, and carryout activities designed for older adults at Bay Ridge Center and Connects locations. She/he will be responsible for, but not limited to:
- Handling case assignments, draft service plans, review case progress and determine case closure; helping clients achieve wellness and autonomy; facilitate multiple care aspects
- Record case information, complete accurately all necessary forms and produce statistical reports; promote quality and cost-effective interventions and outcomes; assess and address motivational and psychosocial issues and adhere to professional standards as outlined by protocols, rules and regulations.
- Membership enrollment, maintaining and updating member files, data entry, outreach efforts
- Assisting members with benefits such as; SNAP, SCRIE, HEAP, SSI, etc..
- Support the Director in developing culturally competent and sensitive programming that welcomes people of all faiths, races, ethnicity, gender orientation and people with alternative abilities.

Qualifications:
- Minimum of B.S in Social Work, Education or similar degree
- Understanding of human service work and its practice
- Experience working with older adults preferred. Experiencing coordinating activities with older adults is also preferred. Excellent leadership and interpersonal skills.
- Organizational and time management to include weekly and monthly reports
- Proficiency in Microsoft Office, Excel, Word and PowerPoint, plus database management experience and the use of Google Apps: Docs, Sheets, Slides and Forms.

Pay: $25.00 per hour
