Care Coordinator – Criminal Justice Focus (Knox)
Duties: In this role, you will provide care coordination services to adult clients with mental health and substance abuse issues, as well as involvement in the criminal justice system. This position is responsible for determining client’s needs and ensuring delivery of needed treatment, bringing services, agencies, resources and other people together to assist a client in achieving established goals, and assisting with the coordination of services for client care, maintaining all required documentation of case management.

Qualifications: Candidates must possess strong written and oral communication skills and the ability to collaborate with other service providers. CMA certification is required. Associates Degree preferred. CPR/BLS required. Basic computer, phone and typing skills are necessary for all positions.
