Black Family Development, Inc. (BFDI), headquartered in Detroit, Michigan, is a 501©3 organization, founded in 1978 by the Detroit chapter of the National Association of Black Social Workers. Our youth- focused, and family-centered organization provides services in Detroit, Wayne County.

BFDI is a CARF accredited, licensed, comprehensive human services organization committed to serving all persons, while specializing in evidence-based counseling, treatment, and advocacy for persons of color. Agency programming includes early childhood resources and literacy development, a 3-tiered juvenile justice service array, mental health prevention and treatment, substance use disorder prevention and treatment, boys and young men of color supportive resources, school-based intervention, and trauma-informed care. Most importantly, our staff are heroes who strive to match passion for service with equal parts best practice to partner with families to achieve their goals.

POSITION: Project Launch Social Worker

DEPARTMENT: Project Launch

REPORTS TO: Project Director or Designee

SUPERVISES: NA

GENERAL RESPONSIBILITIES

The Social Worker will provide clinical counseling and therapy services to children and families with mental health services including direct counseling services, crisis intervention, and coordination of community services/resources. They will assess clients' needs, develop treatment plans, and provide ongoing support and counseling. They may also collaborate with other healthcare professionals to coordinate care and provide referrals to other services and resources. They are responsible for maintaining accurate client records and ensuring that all services are provided in accordance with ethical and legal standards.

DUTIES AND RESPONSIBILITIES

  • Conducts comprehensive, culturally sensitive psychosocial assessments, develops care plans, and provides therapy and crisis intervention services to BFDI consumers.
  • Participates in team meetings and participates in continually improving care based on results.
  • Develops, implements, updates and/or monitors treatment plans for own caseload.
  • Assists in maintaining patient pre-authorizations, completing treatment plans in a timely fashion.
  • Responsible for accurate, timely communication with the support staff and other clinical staff to maintain high quality services, accurate schedules and clinical records.
  • Participates with office on-call schedule.
  • Other duties as assigned.
  • Works under the supervision of the Project Director to provide high quality mental health services.
  • Conducts comprehensive, culturally sensitive bio-psychosocial assessments, develops treatment plans, and provides therapy and crisis intervention services to individuals and families.
  • Supplies information and education about mental health issues to individual patients and to groups.
  • Documents diagnostic assessment, intervention, and treatment data in medical records.
  • Provide support to area school systems with recommendations and attend IFSP, IEP, ARD meetings as needed.
  • Ensure all performance improvement activities are managed and performed according to regulatory and accreditation standards, corporate policies, and related measures.
  • Provide crisis interventions

  • Demonstrates competency in practice and knowledge of current standards of practice.
  • Maintains credentials and performs current practice expectations within established guidelines.
  • Develop and maintain collaborative internal relationships and partnerships with co-workers
  • Develop and maintain favorable external relationships with vendors, contractors, and related resources.
  • Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional and ethical manner at all times.
  • Uphold and ensure compliance and attention to all BFDI policies and procedures as well as the overall mission and values of the organization.

  • Five-points of case management (assessing, linking/coordinating, monitoring, follow-up, and re-assessing).
  • Considerable knowledge of community systems and inter-agency networking

QUALIFICATIONS

  • Minimum Master’s Degree in Social Work or related field; experience in alcohol and other drug treatment/prevention; knowledge of client population and treatment strategies.
  • Mastery of in-home human services, and crisis intervention; individual, family, and group counseling; comprehensive assessment and diagnostic skills; time management; report and case note writing skills; strong engagement and empowering skills; determination to see families excel with provided resources.
  • Ability to accurately complete reports on a timely basis.
  • Ability to process dictation.
  • Ability to maintain, store, organize, and retrieve documents.
  • Proficient in Microsoft Office.
  • Ability to work a flexible schedule.

Job Type: Full-time

Pay: $55,000.00 - $60,000.00 per year
