Patient Navigator (RN) Patient Navigation / Full-time

POSITION SUMMARY: This role involves managing and coordinating Care Coordination services within a health system, focusing on person-centered care for patients in hospital or clinic settings. The position emphasizes collaboration with Care Coordination staff to ensure high-quality services and the development of care protocols alongside clinical leaders.

Requirements

• EDUCATION: Registered Nurse, BSN preferred.
• CERTIFICATION/LICENSES: Current New Mexico Nursing License.
• SKILLS: Knowledge of healthcare policies, ability to coordinate services, strong communication skills, and cultural sensitivity.
• EXPERIENCE: Minimum of two years in a healthcare setting with a focus on quality and care coordination preferred.
  
  

NATURE OF SUPERVISION: Reports to Manager of Population Health and Director of Ambulatory Patient Care Services.

ENVIRONMENT: Involves multiple settings with potential exposure to unpredictable situations.

PHYSICAL REQUIREMENTS: Ability to navigate care settings, handle stress, and demonstrate excellent customer service skills.
