JOB DESCRIPTION: LICENSED PRACTICAL NURSE
Policy
The Licensed Practical Nurse provides skilled nursing care to clients as prescribed by the physician and in a compliance with the regulations as established by the State Board of Nursing, procedures of Bluefox Healthcare and the clients. The LPN works under the supervision of the Registered Nurse Supervisor in the planning and delivery of client care. Assignment of LPN to a client shall reflect the highest standards of nursing practice, shall be proactive, reactive to clients’ needs, be provided in a manner which promoted quality, continuity, and safety of the client care. The LPN may be assigned to give a direct nursing care which requires the skills and or judgment of a Licensed Practical Nurse.
Reporting relationship
- The immediate supervisor of the LPN is the RN Supervisor or the designated RN. The LPN is observed and supervised by the RN Supervisor at least every 30 days
- When on assignment, the LPN is expected to follow the reporting relationship as outlined by the institution’s policy. This chain of command can be expected in most cases. Issues will be firstly directed to the Director of Nursing/RN Supervisor and if he/she is not available or could not resolve the issue then the Administrator will be contacted as a last resort
Requirements (Health)
All Clinical Staff must provide documentation of the following:
- Medical Exam verifying that they are free from infectious diseases and are physically able to perform job functions
- The vaccination with Hepatitis B vaccine, or declination of Hepatitis B vaccination statement. If a declination is desired, the person requesting not to be vaccinated must sign a Hepatitis B Vaccination Declination
- An Annual TB Screening for negative results. If there is a history of positive PPD, a chest x-ray stating that the person is free from disease is required
- Able to lift and bend
Hiring
Prior to employment the candidate must:
- Have a face to face or virtual interview with Bluefox Healthcare LLC staff
- Provide at least two professional references
- Show current state license to practice nursing, original social security card and Driver’s license/ID to Bluefox Healthcare LLC representative (a copy will be made and kept on file)
- Complete and pass a criminal background check conducted by Bluefox Healthcare LLC representative or subcontractor
- Provide a resume with two reference indicating experience in patient care (Bluefox Healthcare LLC representative will verify employer reference)
- Complete a Skill Checklist and demonstrate competency that is observed, documented, and verified by signature of RN Supervisor or RN Designee
- Complete application form and Meet all health requirements
Responsibilities
All duties and responsibilities shall be in accordance with federal, state, and local laws.
- The LPN is responsible for seeking and obtaining sufficient information necessary to function effectively in the health care setting where assigned.
- Specific tasks shall include, but are not limited to the following:
- Provide nursing care through evaluation and implementation of the client’s nursing care plans written by a registered nurse
- Contribute to the supervision of CNA and HHA
- Implement orders for medications and treatments as prescribed by the physician in the medical plan of treatment
- Active member of the health care team, assisting the physician and/or registered nurse in performing specialized procedures in addition to preparing equipment and materials for treatment and observing aseptic techniques
- Demonstrates the ability to provide and assure the carrying out of established nursing procedures utilizing the nursing process, and assuring the maintenance of a safe, clean environment. Provides holistic, age appropriate total patient care demonstrating the ability to assess patient and situation needs and problems
- Demonstrates thorough, concise, and timely documentation, shift reports and verbal communications. Communicates pertinent patient information and unit information to physicians, charge nurse and other appropriate persons while maintaining client confidentiality.
- Efficiently and effectively provides total patient care including patient assessment, cares, teaching, care planning, rehabilitation, medication, treatments, activities of daily living, discharge planning, etc. Completes annual competencies.
