Overview: Bridge City Family Medical Clinic is a Nurse Practitioner owned private medical clinic established in 2003. We are a state-certified, tier 4 patient centered primary care home located in Portland Oregon. We serve a diverse patient population, including a large percentage of LGBTQ patients ranging in age from birth to death. We are seeking a psychiatric nurse practitioner to join our care team using the Collaborative Care Model of integrating mental health services with primary care.
The psychiatric nurse practitioner will provide psychiatric evaluation and medication management for patients in an outpatient setting. Our patients may be typical patients seen in a primary care setting or they may have chemical dependency, emotional or behavioral disturbance and/or other psychiatric diagnoses.
Patients may be seen in office or via tele-health.
Minimum Qualifications:
- MSN or DNP
- DEA
Essential Functions Utilize validated outcome measures throughout treatment process Contribute substantively to interdisciplinary approach and treatment of all clients Participate in on-going evidence-based specialty training
- Utilize electronic health record system (EHR)
- Document all client correspondence within 24 hours and document all patient encounters within 24 hours
- Maintain licensure, DEA certification, malpractice insurance, and adhere to all Oregon state and National Board certification requirements
