Position Summary:
The Wellness Nurse role includes assisting in supervising care staff and communicating with residents and family members about any health care needs. Primary duties will be to assist with the daily care of all residents in accordance with established policies and procedures of the community. The Wellness Nurse promotes hospitality and delivers a high level of customer service at all times. Works closely with Director of Wellness and Resident Care Coordinator to ensure compliance with state and federal guidelines and regulations regarding resident care.
Essential Functions:

Resident Care
- Assesses residents for changes in condition or clinical concerns, whether physical or mental, and communicate those changes to the Director of Wellness, family members and appropriate outside care providers.
- Documents any changes of condition in resident’s medical record.
- Assists in the processing of paperwork for all new admissions, discharges, and transfers of residents.
- Prepares paperwork for MD appointments.
- Coordinates resident/HCP visits by practitioners coming to community. Processes all new orders and follows up post visit.
- Performs all nursing treatments as ordered by HCP for residents on LNS services. Documents appropriately in LNS binder for all services provided.
- Assists in the recording of all relevant information for the Resident’s medical record to include monthly weights and vital signs.
- Responds to resident emergencies, incidents, or accidents. Implements crisis interventions as needed. Completes incident reports and follow up.
- Assists with resident care duties and medication administration as needed to ensure all care and medication services are delivered in a timely manner.
- Other duties as assigned by the Supervisor.
Medication Management
- Assists in the training of new Med-Techs as needed and assists Director of Wellness and Resident Care Coordinator with completing initial competency assessment.
- Reviews EMR daily to ensure compliance with proper documentation.
- Monitors the Nurse Call system and alerts care staff of pending resident calls via communication over radio.
- Monitors the average response time for staff to respond to the emergency call system. Follows up with staff to ensure call pendants are answered in a timely manner.
- Collaborates with the team in care coordination meetings.
- Participates in COVID testing and FLU clinic coordination.
- Reports all medication errors to Director of Wellness immediately.
- Attends and participates in bi-monthly staff meetings.
- Other duties as assigned by the Supervisor.
Staffing Supervision
- Assists in completing regular in-service to staff to ensure that they are kept informed of new information or changes in policies that contribute to meeting nursing department objectives/standards.
- Assists in the orientation of new staff.
- Ensures care staff members are in the dining room to help facilitate a smooth dining experience for all residents during mealtimes.
- Ensures that toileting, showering, and laundry schedules are maintained.
- Assists in maintaining all care schedules, making certain that each shift is adequately staffed to provide quality care to residents. Notifies Resident Care Coordinator and/or Director of Wellness of any changes to schedule including call offs and tardiness.
- Attends and participates in bi-monthly staff meetings.
- Assists in formal evaluations of staff and provide corrective counseling as needed.

QUALIFICATIONS

Educational Requirements and Experience:
- High School diploma/GED.
- Current Florida LPN license.
- Current CPR certification.
- Must be 18 years of age.
- Must have dementia/Alzheimer’s experience.
- Previous experience working with seniors preferred.
- Previous experience working in a supervisory role preferred.
Language Ability:
- Ability to communicate effectively speaking the primary language of the residents.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to write in required logs and routine correspondence.
Environmental Adaptability:
- Employees may be exposed to blood and/or body fluids with potential exposure to hazardous materials and infectious diseases.
- Possible exposure to chemicals as identified in the MSDS Manual.
- Exert 50 to 100 lbs. of force occasionally, 20 to 50 lbs. of force frequently, and 10 to 20 lbs. of force constantly to move residents or objects.
- This position requires repetitive bending, lifting, pushing, pulling, reaching, and walking.
Job Types: Full-time, Part-time
Pay: $28.00 - $29.00 per hour
