General Summary: Position requires professional skill, judgment and training to assess the needs of the patient, and to assist the physician in developing a plan of care that meets those needs. * Participates in the completion of the Home Health Certification and Plan of Care under the direction of the physician. * Travels to patient’s homes to conduct skilled assessments, admissions, recertifications, discharge, transfer and other assessments, and to perform skilled treatments and teaching according to the Physician’s Plan of Care. * Completes OASIS Assessments as required under the Medicare Conditions of participation. Coordinates and directs various aspects of the patient’s home health care services as prescribed under the Physician’s Plan of Care. * Maintains communication with the patient and physician in matters concerning the patient’s needs and the plan of care, including but not limited to Ø responses to treatment, Ø changes in condition, Ø compliance with the plan of treatment, Ø changes in environment or support systems, or Ø other issues that may suggest a need for change in the plan of care. * Establishes a nursing care plan based on the Physician’s Plan of Care and Nursing Diagnoses developed from the assessment of the patient. * Assesses the patient condition and reviews documentation routinely and on an ongoing basis to monitor the progress, condition, and care of each patient. * Makes appropriate referrals for allied health care disciplines, and social service as necessary to carry out the goals and directives of the Physician's plan of care. and for durable medical equipment, supplies, oxygen, or other services as ordered by the physician. * Performs Home Health Aide (HHA) orientations as necessary and supervises the provision of services by LVN’s, HHA’s, contract or other staff RN’s, therapists, and social workers, as necessary to carry out the goals and objectives of the Agency and the Physician's Plan of Care, and to protect the health and safety of the client. Caprock Home Health Services, Inc Position Description Revised 01/2018 Communicates and cooperates with other staff members and health care workers to provide the best care possible for the patient and carry out the overall objectives of the agency. * Takes call on a rotation basis. * Cross trains in basic requirements of various Agency programs and third-party payors requiring RN participation, in order to adequately assure access to appropriate nursing care for Agency clients. * Provides backup and support for RN's in various Agency departments or programs as required to assure the health and safety of Agency clients. May be called upon to perform nursing services under Licensed Home Health category of services, including but not limited to, skilled visits, attendant orientations, quarterly or annual assessments, and admissions. * Participates in ongoing training and Quality Assurance/Quality Improvement activities. * Position can be called upon to perform other duties as assigned. Job Skills and Abilities: The ability to exercise independent professional nursing judgment in compliance with a Physician’s Plan of Care and applicable regulations is an essential requirement of the position. Must Practice in compliance with the professional standards and regulations of the Texas Board of Nurse Examiners. Must possess comprehensive knowledge of general nursing theory, technique, and practice plus ability to effectively work with ancillary personnel. Must have propensity for problem solving. Position requires the ability to learn, manage, and ensure compliance with various regulations and guidelines, including but not limited to state Home and Community Support Services licensing requirements, Medicare Conditions of Participation, and various THHS nursing programs. Knowledge of Medicare OASIS and Medicaid RUGS assessment techniques, terminology, methods, and forms. Strong written and oral communication skills required. Must develop proficiency with, and use assigned point of care software for recordkeeping and client care management. Must be able to communicate, i.e., speak, read and write in English. Ability to communicate in Spanish is a plus. Qualifications: Physical: Physical ability to travel without assistance to client’s home, to lift and carry appropriate medical supplies into and out of a vehicle, into and out of the home. Requires physical strength and manual dexterity sufficient to safely carry out all physical activities required for accurate and complete patient assessment and treatments. Examples of activities for which physical strength may be required include turning and partial lifting of patients for appropriate assessment of all body parts, operation of a hoyer lift, support of patients with various musculoskeletal disorders with rising, sitting, Caprock Home Health Services, Inc Position Description Revised 01/2018 standing and ambulation activities. Strength, stability, and manual dexterity must be sufficient that the nurse can safely and effectively perform delicate procedures such as intravenous placements and blood draws without injuring anyone. Education, Training, and Professional Credentials: A graduate of an approved school of professional nursing who is licensed in the state where practicing. Experience: Position prefers one year of experience as a licensed nurse
