Position Summary :
The Community Engagement Coordinator (CEC) for Access & Opportunity focuses on expanding healthcare access and creating pathways to essential services for individuals and families facing barriers to care. This role combines advocacy, education, and outreach to promote Care Alliance Health Center (CAHC)’s services, attract new patients, and connect community members with vital health resources. The position also includes Medicaid-billable services, such as care coordination, structured health education, and social determinants of health screenings.
Key Responsibilities :
Outreach & Relationship Building :
- Conduct grassroots outreach to distinct populations to promote CAHC’s services and programs.
- Build relationships with culturally distinct community leaders, organizations, and residents to promote CAHC’s services.
- Represent CAHC at cultural events and community forums to increase visibility and attract new patients.
Client Support :
Assist clients with navigating healthcare systems, scheduling appointments, and enrolling in insurance programs.Provide structured, Medicaid-billable health education programs tailored to distinct communities (e.g., chronic disease prevention, maternal health, behavioral health awareness).Conduct culturally competent social determinants of health screenings and link clients to supportive resources (e.g., food security programs, transportation, employment assistance).Offer care coordination services under Medicaid guidelines to improve healthcare access and continuity of care.Provide language access support and referral to interpretation services when needed.Community Engagement & Patient Attraction :
Design and execute community events, such as workshops and focus groups, to engage distinct communities.Identify opportunities to promote CAHC services and attract new patients.Participate in public health campaigns that address disparities within minority populations.Data & Reporting :
Track engagement efforts and document billable screenings, assessments, and care coordination services.Conduct Medicaid-approved health risk assessments and ensure proper documentation in CAHC’s EHR system.Provide reports on community needs and program outcomes to inform strategic planning.Qualifications :
Education & Experience
Associate or Bachelor’s degree in Social Work, Public Health, or a related field preferred.CHW certification (or willingness to obtain within six months; required for Medicaid billing).CPR / First Aid certification preferred.Certified Healthcare Interpreter (CHI) or equivalent credential preferred if providing interpretation services.Completion of cultural competency and health literacy training preferred.Skills & Competencies :
Strong cultural competency and communication skills to engage distinct populations effectively.Knowledge of Medicaid-billable care coordination and health education programs.Knowledge of local resources available for minority communities, including language access services, culturally specific healthcare providers, and public benefits programs.Proficiency in Microsoft Office and EHR systems (EPIC experience preferred).
