CareNational is looking for an experienced Licensed Practical Nurse in New York City, NY! This is a contract position and ideal candidates will have at least 3 years of recent experience as a Quality Specialist.

Pay Details

• Pay Range: $38.00 - 40.00 / Hour
  
  

Work Setting

• Remote, will need to be local to the New York City NY area to pick up equipment in the office.
  
  

Position Details

• Contract Length: 8 weeks
• Location: New York City, NY
• Monday-Friday 8am-5pm
  
  

Requirements

• Valid NYS Licensed Practical Nurse License.
• 3 years of clinical experience.
• Strong communication skills, including verbal and written communication skills, and strong interpersonal and organizational skills.
• Competence with video conferencing applications such as Teams, Zoom, and Webex.
• Proficiency with health-related computer applications.
• I/DD Experience strongly preferred
• Previous quality improvement experience preferred
• Previous chart review experience preferred
  
  

Responsibilities

• Support healthcare initiatives in providing clinical supports to complex people with I/DD.
• Work collaboratively with the Nurse Manager of Healthcare Management to risk stratify individuals for RN clinical assignment.
• Function as a clinical resource to care managers.
• Perform electronic chart reviews.
• Documents clinical findings and recommendations in the electronic health record within the scope of their license.
• Provide clinical guidance to support care managers with coordinating and providing access to preventative and health promotion services.
• Actively participate in interdisciplinary virtual meetings with care teams as needed.
• Meet all training requirements on time.
• Other duties as assigned.
