Essential Functions and Responsibilities

SUMMARY

The Resident Service Coordinator will connect eligible residents to needed supportive services or medical services which are provided by private practitioners or agencies in the general community. In addition, duties may include formal and informal case management, in which the service coordinator is responsible for assessing service needs, determining eligibility for public services, and making resource allocation decisions. In addition, this position is responsible for fostering an environment in which the residents can live independently and helps to access services available in the community, and designs programs and services to meet the needs and desires of the property’s residents.

Provide general case management (including intake) and referral services to all residents needing such assistance.*

May provide formal case management for a resident when such service is not available through the general public.*

Establish linkages with all agencies and service providers in the community; shop around to determine/ develop the best "deals" in service pricing to assure individualized, flexible, and creative services for the involved resident(s).*

Set up a directory of providers for use by both project staff and residents.*

Refer and link the residents of the project to service providers in the general community, such as: case management, personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurse, preventative health screeners, wellness, and legal advocacy.*

Educate residents on service availability, application processes and procedures, client rights, as well as providing advocacy as appropriate.*

Monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual.*

Manage the provision of supportive services where appropriate.*

May set up volunteer support programs with service organizations in the community.*

Help the residents build informal support networks with other residents, family, and friends.*

May provide training to project residents in the obligations of tenancy or coordinate such training.*

May educate other project staff on issues related to aging and service coordination to help them to better understand, work with, and assist the residents.*

Conducting comprehensive, non-clinical assessments of residents for wellness and social needs. *

Helping residents to identify, access, and coordinate services (such as personal care services), including monitoring of services provided and follow-up communication with service providers. *

Monitoring the receipt and follow through of services, including encouraging and motivating residents to engage with providers and participate in their own care/services management. *

Proactively developing and arranging educational/preventative health programs and services for residents. *

Developing and sustaining partnerships with the Area Agency on Aging, the Aging & Disability Resource. *

Connection, community-based supportive service providers and other community stakeholders. *

Maintaining an up-to-date resource directory with all local service providers. *

Qualifications and Physical Demands

EDUCATION and/or EXPERIENCE

Associate’s (A.A.) Degree in social work, gerontology, psychology or counseling is preferred; or High school diploma or general education degree (GED) with 3 to 5 years related experience and/or training; or equivalent combination of education and experience.

CERTIFICATES, LICENSES, REGISTRATIONSValid Driver’s License and insurance is required. 

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
