Social Worker  Responsibilities :

  • Provide social work services to residents and their families, offering emotional support and guidance.
  • Conduct assessments to identify resident needs and develop individualized care plans.
  • Assist residents in accessing community resources, benefits, and services.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Address and resolve concerns related to resident care and social needs.
  • Maintain accurate and timely documentation in compliance with state and federal regulations.

    Qualifications :

  • Required : Active Ohio LSW license.
  • Experience in long-term care, healthcare, or a similar setting is required.
  • Strong interpersonal and communication skills with a passion for helping others.
  • Knowledge of state and federal regulations related to social work in nursing homes.
  • Ability to manage multiple tasks and work effectively as part of a team.
