Director of Transitional Services

Description

1. Has primary responsibility for Patient Satisfaction (PSAT) for Transitional Rehab patients.
2. Achieves facility census goals and annual budget projections.
3. Serves as a liaison between the facility and referral sources including, but not limited to, hospitals, physicians, community organizations and home health agencies.
4. Develops objectives and strategies to maximize short and long-term admissions utilizing key referral sources.
5. Serves as a liaison between the facility and patients/family members with a specific focus on customer service.
6. Works with interdepartmental team to coordinate screening and admission of patients to the facility.
7. Collects and distributes facility statistics for internal and external purposes.
8. Provides feedback to interdepartmental teams on process improvement and customer service challenges.
9. Conducts appropriate research and analysis of local market and current market trends.
10. Maintains effective and timely communications with facility administrator, designated team members and key referral sources.
11. Takes initiative; looks for opportunities to improve patient experience, validate our differentiation to referral partners and improve our outcomes.

Qualifications

Bachelor's Level Degree in Marketing, Communication or Socia Services.
