Catholic Charities of Los Angeles, Inc. is one of the largest human services providers in California. The agency operates a variety of programs to serve the poor and strives to find permanent solutions to crises by offering clients the tools and resources they need to achieve greater self-reliance and stability in their lives.
The San Pedro Region provides basic assistance with food, shelter, utilities, clothing, transportation, case management and linkages to community resources for the homeless and low-income households. The Region is also responsible for two homeless shelters one of which is a family shelter to assist homeless families, pregnant women, the disabled and the elderly that moves families from crises to stability. The second shelter is a Bridge Housing shelter for homeless single men and women. The region is also responsible for three community centers of which two serve as a food pantry.
The Region is seeking an experienced Release Services Case Manager to be a part of the Time-Limited Subsidy Program that is a critical component of the LA Continuum of Care portfolio of permanent housing programs. The Time-Limited Subsidy program is aHousing First, serving anyone regardless of housing barrier, and practice trauma-informed and harm reduction approaches to their work with people experiencing homelessness. They fill the needs of people who do not need a deeper level of support that can be found in our other permanent housing programs such as Permanent Supportive Housing (PSH).
Time Limited Subsidies is a Housing First, Low Barrier, Harm Reduction, Crisis Response program focused on quickly resolving the crisis of homelessness for eligible participants and assisting them at moving into a permanent housing situation. Time Limited Subsidies provides case management and financial assistance including rental subsidies for a period up to twenty-four (24) months. Time Limited Subsidies case managers use Progressive Assistance to help program participants rapidly obtain and stabilize in permanent housing in either the private rental market or affordable housing market as available.
Responsibilities :
- Experience working as a senior level case manager.
- Ability to encourage participants cooperation as a way of promoting standards of competency, efficiency and accountability.
- Meet with participants who are unhoused once a week to discuss, review and as needed update the housing search plan.
- Research relevant housing topics, and collect and organize relevant housing information for participants and organizational staff.
- Must take the Homeless Management Information Systems (HMIS) training and demonstrate competency with all client data management tools available to the teams, databases, HMIS, and contracts.
- Document participant status in the HMIS database.
- Conduct a minimum of one (1) home visit each month after the participant has been housed.
- Must be able to analyze the need and frequency for home visits.
- Have basic knowledge of Housing-focused case management that support the housing retention
- Ability to assess and reassess participants needs, household budgets, scheduling of appointments and follow up to ensure retention plans are being followed.
