Position Data:
Title: Child Health Services Coordinator
Grade: E8a Exempt ____ Non-Exempt X
Reports to: Division Director
Basic Function or Position Summary:
Coordinates health services (including mental health, dental care and disability services) for children and families enrolled in Head Start, Early Head Start, pre-k and child care services. Salary $20.24 - $21.24/hourly
Security Clearance: Level 3 V,A,C,R
Requirements
Position Responsibilities and Specific Duties:
• Leads Division’s Health Advisory Committee in compliance with Head Start Standards.
• Ensures health and mental health services are offered to customers uniformly at all sites and in compliance with Head Start Performance Standards, OCFS regulations, and NYS Department Health regulations.
• Implements work plan and facilitates service outcome that foster positive health outcomes for children and families.
• Oversees development of day care center Health Care Plans
• Maintains division plans, procedures and protocols related to health services; proposes new division procedures and forms as necessary
• Assists in developing partnerships with local healthcare providers to facilitate positive customer outcomes.
• Ensures customer documentation is compliant with Head Start Performance Standards, OCFS regulations, and NYS Department Health regulations, including but not limited to
o Physicals
o Immunization records
o Health screenings
o Individual Health Care Plans
• Consults on IFSP’s and IEP’s for children with disabilities.
• Provides health education and information to parents on parental rights for children with special needs
• Provides technical assistance, coaching, mentoring and professional development.
• Provides coaching, mentoring, and technical assistance to health care staff and center directors
• Assists with providing and developing trainings and orientation to service area staff.
• For assigned child care center
o Provides oversight, review and approval of health records and health histories.
o Develops and tracks health care plans, including special diets and medications.
o Documents children’s incidents.
o Completes children’s screenings within 45 days of enrollment and initiates referrals for evaluation.
o Facilitates community referrals, including access to a medical home for children and families.
o Facilitates and coordinates services to pregnant women.
o When Center Director is not on-site, acts on their behalf in accordance with OCFS regulations
• Maintains training in compliance with OCFS regulations
• Participates in grant preparation as designated.
• Maintains current knowledge of and assures regulatory and procedural compliance.
• Works with team on PR and recruitment strategies; community relations.
• Maintains current knowledge of and assures regulatory and procedural compliance.
• Develops and keeps procedures and protocols current; implements designated management systems.
• Assists with providing and developing trainings and orientation for service area staff.
• Promotes agency wide integrated service delivery strategies.
• Participates on designated Coalitions.
• Fosters positive relationships with peers, funders, licensors, staff, subcontractors and community partners.
• Monitors identified service area budgets; assists division director to develop new or renewed budgets.
• Facilitates and participates in meetings as designated.
• Fosters ongoing, integrated, comprehensive service delivery model with other COI services.
• Coordinates designated functions of the MOU and MOA process.
• Facilitates standardization of systems and processes within service area and division.
• Utilizes CAPSYS, data, and other tracking and measurement tools to monitor staff performance and service area outcomes; drive improvement and response to need in service area
• Promotes positive agency, division and service area image
• Other duties as assigned.
