Position Data:
Title: Nurse
Grade: E8A Exempt: Non-Exempt: X
Reports to: Center Director
Basic Function or Position Summary:
Provide opportunity for optimum health to families and children through nutrition, health education, screenings and referrals to healthcare providers based on assessment and need. Hours per week 35: Monday - Friday 7:30am to 3:30pm. Salary $20.24 - $21.24/hourly
Security Clearance: Level 3 V, A, C, R
Requirements
Position Responsibilities and Specific Duties:
• Facilitates integration of service delivery between Early Care and Education to eliminate redundancy and ensure highest levels of services are provided to children and families.
• Maintain knowledge and ensure compliance of Early Care and Education.
• Provides oversight, review and approval of health records and health histories.
• Develops and tracks health care plans, including special diets and medications.
• Documents children’s incidents.
• Provides education, training and technical assistance to staff, volunteers and families.
• Works cooperatively with management, staff, and parents to promote positive health outcomes while ensuring compliance.
• Completes children’s screenings within 45 days of enrollment and initiates referrals for evaluation.
• Facilitates community referrals, including access to a medical home for children and families.
• Facilitates and coordinates services to pregnant women.
• Maintains awareness of new developments/trends, which may impact service delivery.
• Manages and material resources by controlling expenses and minimizing waste.
• Maintains knowledge (professional and technical) of current practices through education, training, workshops, conferences, and establishing professional and personal networks.
• Meets or exceeds required training hours.
• Maintains complete information on CAPSYS and mandated health files.
• Maintains and initiates contact with families such as attendance, health information or result of an illness or incident.
• Assists in transitioning of children’s records.
• Administers medication in accordance with Federal and State regulations.
• Performs assessments of children’s health status.
• Promotes positive agency image.
• Participates in the larger team of the agency through committee structures, management systems, planning, policy development and volunteerism as designated.
• Ensures confidentiality and complies with the code of ethics.
• Assist with maintaining inventory and ordering supplies/equipment.
• Maintains consistent professional customer service.
• Job performance incorporates integrated service delivery model while promoting self-sufficiency.
• Other duties as designated by supervisor.

A. Internal: Early Care and Education team, COI management team, staff and customers
B. External: Community agency representatives, funders / partners, and local Health providers

Organizational Responsibilities:
• Ongoing tracking, monitoring and reporting, including health outcomes.
• Assists in policy, procedure and program plan updates
• Participates in organizational committee structures as appropriate
• Participates in organizational and divisional management systems
Knowledge, Skills Required:
• LPN Degree required; RN Degree preferred.
• Knowledge of health and wellness focus beneficial.
• Experience in public health- pre-natal, field beneficial.
• Knowledge of health, nutrition, and local and state regulations beneficial.
• Ability to work as an effective team member/leader.
• Strong computer skills.
• Maintain positive and cooperative relationships internally and externally.
• Excellent organizational skills including documentation and charting.
Special Requirements, if any:
• Ability to maintain Nursing Licensure.
• Completes all OCFS requirements.
• Maintain a current Driver’s license.
• Ability to travel within New York State and occasional trips outside NYS.
• Ability to work flexible hours.
• Lifting requirements: up to 50 lbs.
• Must be able to provide consistent even tempered customer service at all times.
