POSITION SUMMARY

Provide relevant nursing and support services in our mental health office.

Participate in care team to develop and implement plan of care following Patient Centered Medical Home model. Provide care for patients with acute, chronic, and/or terminal illnesses. Within the scope of Maryland Nurse Practice Act, the Registered Nurse may be assigned specific responsibilities for certain direct patient services. Active participation with interdisciplinary teams, committees, program evaluations, research, and education. This position will report directly to the Site Manager.

PRIMARY ACCOUNTABILITIES

Achieve Results

  • Improve patient outcomes, and deliver quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction.
  • Participates in quality strategies to evaluate compliance with standards and identify opportunities to improve patient outcomes.
  • Perform an assessment based on the patient’s diagnosis, reason for entry into system and care needs. Utilize the nursing process to plan, perform, delegate and evaluate the delivery of nursing care.
  • Stay current with knowledge, concepts, practices and skills appropriate for the clinical area assigned.
  • Have working knowledge of PCMH process and goals.
  • Participate in daily huddle with PCMH care team staff to determine patients who are at high risk and/or are in need of PCMH care management.
  • Conduct independent assessments of patients’ medical, behavioral, pharmaceutical, and social needs and notify PCMH nurse team leader to initiate PCMH process when needed.
  • Ensure all patient charts and related documentation are maintained current and relevant at all times.

· Acknowledge patients’ rights by maintaining HIPAA guidelines at all times.

Operational Excellence

  • Ensure all care and nursing practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, Joint Commission accreditation, NCQA’s PCMH standards and guidelines and related professional standards.
  • Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines.

Relationships

  • Develop and maintain favorable internal relationships, partnerships with co-workers, including clinic managers, nursing staff, physicians, business office staff, and all members of the PCMH care team.
  • Develop and maintain favorable external relationships with vendors, contractors, referral agencies and related resources.
  • Contribute to team success: Support established goals and objectives.

Stewardship and Professionalism

  • Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times.
  • Uphold and ensure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization.

PRIMARY TASKS & DUTIES

  • Provide direct care to patients as assigned and qualified to perform.
  • Assess, review, and develop nursing plans to ensure maximum patient outcomes.
  • Provide counsel, support, advice, and support to patients and their families.
  • Complete all patient charting and ensure all patient records are current and complete.
  • Participate in PCMH care team meetings to develop patient care plans and determine education and resource needs of patients.
  • Provide patient and/or family education regarding medications, self-management, interventions, goals, and other topics as determined by care team.
  • Attend meetings, patient conferences, planning sessions, related to quality assurance, patient care, and other related topics within the clinic.
  • Attend seminars and maintain all licensure requirements for continuing education and best practices.
  • Participate in quality strategies to evaluate compliance with standards and to identify opportunities to improve patient outcomes.

ESSENTIAL FUNCTIONS/ KEY COMPETENCIES

  • Demonstrate high degree of knowledge and competency in the practice of medicine and associated charting requirements.

  • Strong attention to detail and accuracy.
  • Ability to utilize computers for data entry and information retrieval.
  • Excellent verbal and written communication skills.
  • Ability to implement, and evaluate operational and administrative processes.

POSITION REQUIREMENTS

Education

  • Graduation from an accredited nursing program
  • Bachelor’s degree preferred
  • Current, relevant state licensure as a Registered Nurse. New graduates must complete the state board examination within 90 days of graduation.

Experience

  • Demonstrated success in providing clinical care and nursing support within an organization of comparable pace and complexity.
  • Demonstrated success in managing difficult patient/customer situations.
  • Significant knowledge of medical practices and procedures within a primary care environment.
  • Demonstrated success and experience with direct patient services and the proper knowledge and use of medical devices and equipment.
  • Demonstrated success and familiarity with tools, technology, and systems typical found within most progressive health care environments (i.e. personal computer skills, spreadsheets, word processing, patient records systems, EMR systems, etc.)
  • Familiarity with Patient Centered Medical Home model’s purpose and goals.
  • Acknowledge patients’ rights by maintaining HIPAA guidelines at all times.

Physical/ Environmental

  • Occasional lifting of 40 – 50 lbs and pushing of 5-20 lbs
  • Sitting, standing and walking approximately 80% of the time
  • A medium to high level of manual dexterity required
  • Bending and reaching approximately 20% of the time
  • Normal accessibility and mobility throughout the region required
  • Normal overtime/extended work hours

Job Type: Full-time

Pay: $28.00 - $42.00 per hour
