Case Management Associate

Our Mission:
The Chester County Food Bank seeks to create a food secure Chester County. We mobilize and educate our community to ensure equitable access to nutritious and familiar food. Chester County Food Bank (CCFB) is a countywide, non-profit organization located in Exton, PA that has been in existence since 2009. CCFB works with over 100 community partners to distribute over 4.5 million pounds of food annually to Chester County residents. We provide educational opportunities through programs to engage the community in hands-on learning experiences.
Position Summary:
The Case Management Associate supports community members by assisting with SNAP (Supplemental Nutrition Assistance Program) enrollment, answering questions, and connecting individuals and families to essential resources. This role also conducts community outreach to raise awareness of services, build partnerships, and ensure underserved populations have access to critical support programs. Works cross departmentally to deliver high quality service to neighbors across Chester County.
Essential Functions:
The essential functions include, but are not limited to the following:
· Guide community members through the SNAP application process, ensuring accurate completion and submission. Answer questions related to eligibility, benefits, and program guidelines.
· Conduct outreach activities/events coordinating with community partners throughout Chester County.
· Utilize COMPASS to assist community members with additional social assistance programs they may qualify for.
· Organize records, files, and spreadsheets and keep detailed notes on the client application process and interactions to ensure accuracy.
· Collaborates across various Food Bank departments to assist neighbors with accessing resources and food related services.
· Connect individuals and families to additional resources, such as food assistance programs, housing support, and other community services. Maintain up-to-date knowledge of local resources to provide accurate referrals.
· Other duties and responsibilities as assigned.
MINIMUM QUALIFICATIONS (Knowledge, Skills, Abilities):
· Bachelor's degree in social work, public administration, or related degree.
· At least one year of prior experience.
· Working knowledge of community and government resources and ability to forge strong relationships.
· Knowledge of assistance programs.
· Personal transportation and ability to travel around the Chester County region.
· Ability to work a flexible schedule.
· Compassionate, friendly, and positive work ethic.
· Ability to follow written and verbal instructions.
· Proficient knowledge of Microsoft Office (Word, Excel, PowerPoint).
· Valid Driver’s License.
Preferred Qualifications:
· Bilingual English/Spanish
· Experience working with diverse populations.
WORK SCHEDULE: The position is primarily Monday-Friday during normal business hours, but the flexibility to work evenings and weekends is required.
Most of the year, the work is in an indoor environment, protected from weather conditions but subject to some temperature fluctuations. Periodically during the year, the worker is involved in outdoor activities and subject to temperatures below 60 degrees F and above 80 degrees F. Outdoor elements such as precipitation and wind possible. On occasion, the environment may be noisy and distracting.
