The position of Family Support Counselor (Bilingual Spanish) (Infant-Toddler - HVP Programs) will pay $27/hour.
Additionally, the lifespan compensation range of this position is: $27/hour - $30.19/hour.
Must have Covid-19 Vaccine and Booster.
Learn more about the Job:
This role involves Orange County home visitations and will require driving to visits. Visits are held in-person and by video at this time. Mileage is reimbursed at the IRS rate.
Th program serves families with a child 0-3, using the Parents As Teachers (PAT) curriculum. The Family Support Counselor is responsible for implementing a community-based prevention effort for families. This includes in-home or in-office support; in-home parent education classes; group parenting classes; case management; and assisting families with resources within the community. The Family Support Counselor may assist families in crisis to access resources, coordinate information for participant referrals, compile and maintain records, and prepare reports.
Job Responsibilities:
Prevention Efforts
- Work directly with families to provide support and parent education using Parents As Teachers (PAT) curriculum
- Provide group parenting classes in the community
- Provide assessments, developmental screenings
- Provide family assessments; recommend specific resources;
- Participate in goal setting with clients
- Provide referrals and follow up to resources
- Assist families in crisis situations such as immediate need for shelter, childcare, health care, etc. by providing proper referral and case management
- Advocate for clients as needed; i.e. problems with Cal-Works, Medi-Cal and other systems that clients interact with
- Translate materials for clients and the agency/collaboration
- Attend community meetings and/or agency collaboration meetings as assigned
- Attend weekly supervisions
- Attend bimonthly staff meetings and other meetings
- Attend mandatory trainings, as well as other trainings (minimum 4 hours per month)
- Assist families in identifying/linking to appropriate community resources
Case Management
- Meets service provision requirements of Targeted Case Management (TCM) program
- Meets on-going comprehensive case management requirements for providing the following TCM ServiceComponents
- Conduct and document needs assessment
- Development of a Written Individualized Service Plan
- Linkage, Consultation, and Referral including follow-up within 30 days
- Follow up / Periodic Review
Documentation
- Maintain electronic files on all clients in Visit Tracker, and other records/documents as required by the program, agency, or funding source
- Meet documentation requirements in relation to the TCM program, TCM Encounters, and the above noted TCM Service Components
Community
- Maintain high profile in the community
- Maintain/provide current resource referrals within the community
- Develop/maintain effective working relations with all individuals and agencies impacting Program within the community
- Assist Program Supervisor with developing and implementing community outreach programs
General
- Maintain confidentiality in matters of agency operations, personnel, and clients
- Maintain positive communications with all agency staff, clients, and volunteers
- Exercise good judgment in performance of duties and responsibilities
- Support existing agency policies, principles, and mission
Job Qualifications:
- Bachelor's degree in social work, human services, child development, behavioral health or related field, or equivalent educational and work experience.
- Two or more years of experience in working with children and families and/or parenting community outreach with low-income moderate to high-risk families
- Knowledge of child development and community resources desirable
- Ability to meet deadlines required by the program
- Ability to work in a multi-cultural environment
- Bilingual English/Spanish required
- Good organizational, follow-through, and verbal/written communication skills
