The Program Nurse delivers care to children using the nursing process of assessment, planning, intervention, implementation and evaluation. Overall, the Program Nurse contributes to the CHS high performance culture by exhibiting our values and providing quality results that position CHS as the leader in delivering proactive behavioral health, case management, community and early childhood solutions for children and families

Primary Job Functions

1. Deliver care to children using the nursing process of assessment, planning, intervention, implementation and evaluation.

  • Assess the full scope of client needs and communicate findings to the physician.
  • Under the supervision of the physician, collaboratively develop and implement a plan of care that includes physical and mental health, nutrition and educational needs.
  • Continuously evaluate the plan of care and revise as needed.
  • Coordinate client care with other medical providers.
  • Consult with the program staff and recommend improvements for nutrition, sanitation, health monitoring and child development activities.
  • Provide telephone triage to program staff for minor child medical incidents and traumatic issues.
  • Collect and document current health status, medical history and BHOS services.
  • Document accurate and ongoing assessment of client status which reflects nursing interventions, client response to care provided, client needs, problems, limitations and progress toward goals.
  • Monitor provision of medication.
  • Staff cases with supervisor.
  • Plan and provide health/nutrition/sanitation related training to program staff or foster parents.
  • Review menus and make recommendations for improved nutrition.
  • Participates in BHOS treatment team meeting.

2. Contribute to a positive, engaging work environment.

  • Develop a strong knowledge base and stay current on job-related issues and trends.
  • Participate actively in departmental meetings, training and education, as well as the quality process.
  • Comply with CHS’s code of conduct, policies, procedures and other obligations.
  • Assist with training other team members and providing back up when necessary.
  • Pick up projects on the fly; perform other duties as assigned from time to time.
  • Demonstrate the CHS Common Bond values in the performance of all job duties.

Job Qualifications

Education, Licenses & Certifications:

  • Associate degree in Nursing from an accredited university, required.
  • Licensure as a Registered Nurse in the State of Florida, required.
  • Florida Driver’s License within 30 days from hire required, with daily access to a reliable and insured vehicle.

Experience:

  • Minimum of four years of experience as a registered nurse in direct patient care, required.
  • Four years of experience in pediatric or emergency patient care, preferred.

  • CHILD NUTRITION AND DIETARY NEEDS | COMMUNITY RESOURCES | INFANT AND CHILD HEALTH | ABUSE AND NEGLECT RELATED ISSUES

Skills and Proficiency in:

  • Planning, project management, organization and time management.
  • Computer systems and MS Office, including Word, Excel and Outlook.
  • Oral and written communication, including presentation and platform.
  • Collaboration, teamwork, consulting, facilitation, coaching and mentoring

Ability to:

  • Act decisively to protect client.
  • Communicate positively with a broad range of children and adults.
  • Perform at a high level of autonomy, with general supervision.
  • Perform under strong demands in fast-paced, diverse, sometimes ambiguous environments.

  • Meet critical deadlines, while maintaining attention to detail, accuracy and quality.
