Requirements:
- Bachelor's degree in Social Work, or related field, and active Ohio Social Work license (LSW) preferred.
- Two or more years working with children and families preferred. Foster care experience strongly preferred.
Responsibilities:

- TEAM first.
- Assist with intakes, facilitating placements of foster youth in assigned homes.
- Conduct regular home visits, meeting the needs of foster youth and family.
- Provide excellent customer service to foster families, placing agencies, including but not limited to timely documentation, facilitate transportation, etc., as needed.
- Provide case management services to help youth remain n the home and/or community, according to the needs of the child.
- Provide and/or coordinate mental health services for foster children and/or family.
- Participate in team meetings, clinical supervision, peer/case review, training activities, recruitment activities, etc., as needed.
- Timely completes and submits all required clinical documentation on assigned youth in electronic health record system.
- Rotate on-call duties, which includes after-hours crisis management for all youth and handling referrals.

This position is contingent upon the completion of a satisfactory background check and drug test, satisfactory reference checks (emails were previously sent to the 3 references listed on your application), as well as the completion of a satisfactory Department of Justice National Sex Offender search and Ohio Alleged Perpetrator Search, as required by our licensing agency.
