What are the responsibilities and job description for the Supervisor, Case Management/Housing Placement, Friends of the Homeless Shelter position at Clinical & Support Options?
CSO is looking for a Supervisor for Case Management and Housing Placement for our Friends of the Homeless (FOH) Shelter in Greenfield.
This position will oversee all case management services to shelter and housing clients through a collaborative process that assesses, plans and coordinates the options and services that will meet an individual’s needs and support their success in obtaining and/or maintaining housing. This position manages Housing Navigators that work with Friends of the Homeless (FOH) shelter guests to develop housing plans and assist in their housing search for safe, permanent housing. This position also supervises the case manager for housing units, and leads the life skills curriculum for shelters.
As a Case Management and Housing Placement Supervisor:
You Will:
- Hire, train, and supervise Case Management/Housing Navigation staff for FOH North shelter/housing programs
- Ensure timeliness of initial & ongoing case management assessment, housing navigator checklist, and credible documentation to determine eligibility and needs for services, supports and housing
- Oversee the process for client housing plans and action steps, from obtaining needed documents and benefits, to completing housing search
- Manage a small caseload of shelter guests’ housing navigation, including tracking client progress, addressing barriers, engaging clients in their housing plans, and follow up on next steps
- Manage priorities with caseload, and staff oversight, and utilize supports and services as needed for clients
- Support & ensure the planning of workshops, life skills programming, and other activities provided in the FOH Resource Centers that pertain to client goals, such as budgeting, job search, employment search, how to be a good tenant, anger management, housemate pairing and other shared housing tools, and completing housing search
- Support some day to day operations/coverage of the shelter sites, including some travel and time at different locations
- Participate in and prepare for all relevant meetings pertaining to clients, such as Coordinated Entry case conference meetings (including YYA and Veteran as needed), as it pertains to prioritizing and referring eligible clients for permanent supportive housing
- Complete vulnerability assessments, as well as chronic verification and disabling condition verification, as needed with clients
- Assist the Program Director with implementation of behavior contracts and client accountability
- Complete Outcome Summaries and other documentation in a timely manner
Your Background:
- Associate’s or Bachelor’s degree in a related field (preferred)
- Two to five years of supervisory experience
- Experience working with homeless or at-risk population (preferred)
- Computer proficiency
- Valid Driver’s license and reliable transportation
