DEPARTMENT: Adult Services
POSITION: Hourly/Exempt ____ Part-time
BASIC FUNCTION:
The provision of quality authorized services, in the individual's private living environment to program
participants enrolled in the Supportive Living Program with intellectual/developmental disabilities and/or
physical disabilities.
NATURE AND SCOPE:
This position reports to and works cooperatively with Coordinator of Independent Living. This position
works closely with other Adult Services Direct Support Professionals to provide high quality coordinated
services to program participants. This position interacts regularly with Program Coordinator of
Independent Living, Clinical Manager of Adult Services participants and their families. It also interacts
with staff from social service agencies and health care facilities which provide service to our program
participants. This position interfaces with other Community Care Connections (CCC) staff and the
general public. Through the provision of services, program participants will acquire, maintain or improve
skills necessary to live more independently; or to be more productive and participatory in community life.
RESPONSIBILITIES:
• Prioritize the empowerment, choices, rights and safety of people with disabilities
• Staff are expected to provide support services in a compassionate, respectful manner that
recognizes and honors individual choices.
• Work closely with Coordinator of Independent Living, Program Specialists, Clinical Manager of
Behavior Supports , and other DSP’s to develop and implement quality individual support plans
to support growth and independence of persons enrolled in the program
• Work independently and collaboratively to provide direct service to program participants in a
variety of settings to support skill development and independence
• Provide direct service in a variety of settings to increase opportunities for and assist program
participants in achieving desired outcomes as outlined in their Individual Support Plan (ISP)
• Teach, model and coach, rather than do for the participants, to the extent each participant and
situation allows.
• Obtain and maintain CPI Nonviolent Crisis Intervention certification
JOB DESCRIPTION
Direct Support Professional
(Supportive Living)
• Accurately and thoroughly complete all necessary documentation and training in accordance with
CCC policies and procedures, funding stream regulations, and program participant’s Individual
Support Plan
• Report and document any observed changes in program participant’s health status /condition
immediately to the Coordinator of Independent Living or the Clinical Manager of Adult Services-
Behavior Supports
• Build and maintain strong working relationships with other support staff to ensure participants’
interests, dreams and challenges are communicated to their ISP team
• Build and maintain strong working relationships with Behavior Supports to cooperatively ensure
the best possible outcomes for the participants behavior modification
• Follow CCC policies and procedures
• Cultivate and implement effective communication skills
• Maintain HIPAA by following all security procedures to protect PHI and by immediately
reporting any suspected or actual privacy or security incidents.
• Maintain a thorough understanding of program requirements, policies and procedures, and
licensure/regulatory requirements
• Ensure that program participants’ needs are met in a safe manner using appropriate equipment in
accordance with their ISP and CCC policies and procedures.
• Immediately report any concerns, specifically those of abuse, neglect, fraud or waste
• Properly operate a vehicle lift, use tie downs or straps to ensure the safety of the program
participant
• Complete and maintain certifications, as required
• Other related duties as required
