POSITION TITLE:
Primary Care Physician
POSITION SUMMARY:
The Primary Care Physician is responsible for providing exceptional medical care to a unique panel of patients, diagnosing and treating illnesses, and developing treatment plans in collaboration with other Primary Care Team members. The desired candidate has a pleasant and welcoming personality and will support CHCFC’s mission, vision, and values, and contributes to a work environment that is caring, collaborative, and innovative.

ESSENTIAL FUNCTIONS:
- Providing primary medical care to a unique panel of patients:
- Conducting physical examinations and medical assessments
- Diagnosing and treating illnesses and injuries
- Developing and implementing treatment plans
- Prescribing medications and other treatments
- Providing advice to patients regarding disease prevention and health promotion
- Collaborating with other healthcare professionals to provide comprehensive care
- Seeking appropriate consultation and clinical supervision as needed from colleagues, including transfer of patients exceeding professional skills, abilities, or comfort.
- Attending health center staff clinical team meetings
- Providing cross coverage for group practice patients during colleagues’ time off
- Documenting all patient care appropriately and legibly in the medical record
- Providing supervision for up to four mid-level providers within the practice
- Providing weekend, holiday, and night on-call coverage per group rotation schedule
- Performing other duties as required or assigned
Education/license/certification:
- Medical degree from an accredited institution (M.D. or D.O.)
- Valid medical license
- Board certification in a relevant specialty
- Primary care experience strongly preferred
- Experience in diagnosing and treating a wide range of illnesses and injuries
- Excellent diagnostic and treatment skills
- Exceptional communication and interpersonal skills
- Ability to work harmoniously with ethnically, culturally, racially, and gender diverse staff and patients.
- Proficiency in using electronic medical records and other healthcare technologies.
- Proficiency in written and spoken English. Bi-lingual English/Spanish, English/Haitian Creole, or English/Russian preferred.
