POSITION SUMMARY
Provides information and education on Cal MediConnect managed care program and to Medi-Cal beneficiaries, community based organizations and the general public by cultivating and sustaining positive working relationships with community organizations serving Medi-Cal beneficiaries.
COMPLIANCE WITH REGULATIONS:
Works closely with all departments necessary to ensure that the processes, programs and services are accomplished in a timely and efficient manner in accordance with CHG policies and procedures and in compliance with applicable state and federal regulations including CMS and/or Medicare Part D, DHCS and DHMC.
RESPONSIBILITIES
- Provides information about CHG by conducting presentations and outreach to staff and clients of community organizations, medical, dental, and optometry offices.
- Promotes CHG's Cal MediConnect program by organizing and participating in community health fairs and other events.
- Supports the department with the supplies inventory by assisting the Community Services Supervisor in maintaining the supplies cabinet fully stocked and the storehouse well organized.
- Supports team effort by performing other duties as assigned or requested; participating in meetings and on committees; attending work shops, training and seminars; continuing professional development; networking; following corporate, division, personnel and safety policies and procedures.
- Maintains product and company reputation and contributes to the team effort by conveying professional image and accomplishing related tasks; participating on committees and in meetings; performing other duties as assigned or requested.
Qualifications
Education:
- High school diploma.
Experience/Skills:
- Current CA driver's license, proof of auto insurance and working car.
- 1 year successful community outreach experience, preferably related to Cal MediConnect and/or Medi-Cal managed care.
- Knowledge of Medi-Cal managed care plan benefits and eligibility requirements and Community Health Group programs and services.
- Familiarity with community social service organizations and groups.
- Excellent oral communication skills.
- Bilingual ability in Spanish preferred.
- Self-directed; ability to recognize and capitalize upon opportunities to promote CHG programs and services in many different venues.
Physical Requirements:
- Constant driving within SD County
- Public speaking.
- Will be required to work evenings and/or weekends and to carry needed equipment for community events, in personal car (e.g., canopy, helium tank(s), materials, etc.).
