POSITION SUMMARY
The Team Assistant for Palliative Care provides administrative support to the Palliative Care Team.
ESSENTIAL FUNCTIONS
- .Answer phones, route calls, take messages and answer questions as appropriate
- Enter Palliative Care referrals into the EMR and communicate status with patients, families and referral sources in a timely manner
- Assist Palliative Care Team with patient care needs, including DME orders, lab orders, and follow-ups, etc.
- Complete appropriate Palliative Care workflow and scheduling in the EMR
- Complete daily admission reports, triage reports and other data collection as needed
- Assist in clinical functions as needed and assigned to ensure proper billing, including insurance eligibility and verification checks when needed
- Perform a variety of typing, including memos, letters
- Conduct month end close such as billing error corrections and physician visits
- Provide reception desk/phone coverage as needed
- Perform miscellaneous job-related duties as assigned.

POSITION QUALIFICATIONS
- High School diploma required; some college preferred
- 1 year experience in a clerical setting
- Experience in a healthcare setting
- Typing and PC skills required

Work Environment Requirements
While performing the duties of this job, the employee may work in various locations (homes, facilities, hospitals, Inpatient Units) and in various conditions with possible exposure to blood and bodily fluids and to infectious diseases. The job requires the ability to work a flexible schedule; ability to travel locally; PRN emergency call. While performing the duties of this job, there may be exposure to outside weather conditions when traveling between sites. Patients’ home environments are variable regarding heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment is in a controlled atmosphere building.
