What are the responsibilities and job description for the RN Charge Nurse position at Community Nursing Services?
Community Nursing Services, "CNS," in Tooele is looking to hire a Charge Nurse / Clinical Care Coordinator (CCC).
The CCC will work directly with the Hospice and Home Health Care teams in the greater Tooele Valley. CCC's are responsible for the intake and triage assessment of client referrals, coordination of clinical services and procedures, and is responsible for supervision of clinical staff. The CCC also acts as a resource person/liaison for patients, staff, and the community.
Compensation: Starting from $75K
Days for the Week: Monday - Friday
Hours: 8:30am to 5pm.
Location: Hybrid, working from the home office, in home care and at home.
Other: Administrator on Call expectations of one weekend a month (there is extra compensation for these shifts)
Responsibilities:
- Medicare Oasis reviews
- Intake and assessment of client referrals
- coordination of clinical services and procedures
- Supervision of clinical staff
- Resource person/liaison for patients, staff, and the community
- Manage schedule
- Conduct IDT meetings
- Education for team
- Hospice billing
- Patient care satisfaction surveys
- Liaison with long term care facilities
Skills:
- Excellent organizational skills
- Strong leadership ability
- Thorough knowledge of community resources
- Functional use of PC and computer input devices
- Strong Interpersonal skills, including good telephone etiquette
- Spanish speaking a plus
Requirements:
- Licensed Registered Nurse
- 3 years Nursing experience
- 1 year home health, public health, community health or related experience preferred.
- 2 years supervisory/management experience
