Role Description
We are seeking a dedicated and compassionate Certified Physical Therapist to join our Adult Developmental Day Treatment team to provide skilled physical therapy services to adults with developmental disabilities in a center-based setting. The Community School provides day habilitative services to a tight-knit group of adults - many who have significant needs for physical therapy treatment. This position is especially rewarding due to the direct impact our therapists have on improving quality of life.
The successful candidate will be responsible for performing evaluations, developing care plans, and overseeing individualized therapy programs aimed at enhancing mobility, improving physical function, and promoting independence. We provide administrative support to manage service referrals, billing, and retrospective reviews so your focus can be on providing direct one-on-one treatment. All treatment is provided in our center - no need for travel. This is a day habilitation setting that provides transportation to our clients who are in our center from 8:30am-2:30pm Mon-Fri. No set appointment times or no shows. We offer flexible scheduling - easily adjust your day to fit your needs while making a huge difference in the lives of individuals with developmental disabilities. This is a contract position.
Responsibilities
- Assessment and Evaluation:
- Perform thorough evaluations of physical capabilities, range of motion, strength, balance, and coordination for adults with developmental disabilities.
- Identify functional impairments and develop treatment goals based on individual needs.
- Individualized Treatment Plans:
- Design and implement personalized physical therapy programs to improve mobility, strength, and overall physical health.
- Adjust treatment plans based on ongoing assessments and patient progress.
- Therapeutic Interventions:
- Provide hands-on therapy to improve joint mobility, strength, posture, and flexibility.
- Utilize a variety of techniques such as manual therapy, therapeutic exercises, and modalities to promote healing and enhance functional capacity.
- Education and Training:
- Educate patients and caregivers on techniques to improve mobility, prevent injury, and enhance daily living activities.
- Provide instruction on home exercises and assistive devices to ensure patient progress outside of therapy sessions.
- Collaboration:
- Work closely with a multidisciplinary team, including other therapeutic professionals, caregivers, and family members, to ensure comprehensive care and optimal outcomes.
- Documentation:
- Maintain accurate, detailed records of patient evaluations, progress notes, treatment plans, and outcomes.
- Ensure compliance with legal, ethical, and organizational documentation requirements.
- Advocacy and Support:
- Advocate for the needs of patients with developmental disabilities, ensuring that they have access to appropriate services and supports.
- Support patients in their personal goals, encouraging independence, mobility, and overall well-being.
Qualifications & Experience
- A degree in Physical Therapy from an accredited institution and current license is required.
- Must be enrolled as a Medicaid Provider
- Previous experience in a clinical setting is preferred but not mandatory; new graduates are welcome to apply.
- Experience in working with individuals who have developmental delays is highly preferred.
- Strong interpersonal skills to effectively communicate with patients, families, and team members.
- Ability to work independently as well as part of a multidisciplinary team.
