Job Overview
The MDS Coordinator plays a crucial role in ensuring the delivery of high-quality patient care through effective assessment and documentation processes. This position involves coordinating the Minimum Data Set (MDS) assessments for residents in a healthcare facility, ensuring compliance with regulatory standards, and facilitating interdisciplinary collaboration. Duties
- Conduct comprehensive assessments of residents to complete MDS documentation accurately and timely.
- Collaborate with nursing staff and other healthcare professionals to gather necessary clinical information for assessments.
- Ensure compliance with federal and state regulations regarding MDS submissions and quality assurance standards.
- Participate in care planning meetings to discuss resident needs and coordinate appropriate interventions.
- Monitor changes in residents' conditions and update MDS documentation as necessary.
- Provide education to staff on MDS processes, coding requirements (including ICD-9), and best practices for documentation.
- Facilitate communication between departments to ensure continuity of care, particularly for patients transitioning from critical care or home care settings.
- Maintain accurate records and documentation related to MDS assessments, quality measures, and resident outcomes.
Requirements
- Proven experience in a healthcare setting, preferably with knowledge of critical care, PICU, or Level II Trauma Center operations.
- Strong understanding of anatomy knowledge, discharge planning processes, and hospice care protocols.
- Familiarity with MDS regulations and standards is highly desirable.
- Proficiency in using electronic health record systems for documentation purposes.
- Excellent communication skills with the ability to work collaboratively within an interdisciplinary team.
- Detail-oriented with strong organizational skills to manage multiple assessments efficiently.
- Ability to adapt to changing priorities in a fast-paced environment while maintaining high standards of patient care.

Pay: $30.00 - $40.00 per hour
