A DAY IN THE LIFE OF A CARE COORDINATOR
The Care Coordinator plays a pivotal role in supporting patient care by organizing, coordinating, and facilitating all aspects of healthcare management. Reporting to the Medical Director, the Care Coordinator ensures seamless communication between patients, providers, and the care team. This position is responsible for assisting patients with scheduling, completing paperwork, and navigating their treatment plans. Additionally, the Care Coordinator educates patients on available resources and options for managing their health. In collaboration with the clinical team, this role serves as a direct backup to the Medical Assistant, providing hands-on support in patient care, including routine clinical tasks when needed.
QUALIFICATIONS FOR A BEHAVIORAL HEALTH THERAPIST
- Bachelor's degree primarily in social, behavioral, human services, or human-related fields.
- The employee is required to be able to safely operate a motor vehicle and be able to obtain a license.
- Current American Heart Association BLS card
- 1 years' experience
- Knowledge of billing, coding, and medical terminology
- Prior experience providing a high level of customer service in a fast-paced environment, including handling confidential data with discretion
- Proficient in speaking, reading, and writing both English and Spanish.
- Demonstrated ability to multi-task, plan and prioritize.
- Demonstrated experience in an office setting.
- Ability to work with all levels of staff and management.
- Proficient in Microsoft Office.
WORK SCHEDULE
This full-time Care Coordinator position works a typical schedule of 8 AM - 5 PM, Monday to Friday.
SALARY Based on Experience, Licenses, and Education.
$25.00 - $27.88/hr. Bachelor with Agency Affiliation License
$26.44 - $31.25/hr. Master's with Associate License
$29.81 - $33.65/hr. Master's with License
