Constellation Hospice is seeking a Director of Clinical Services for our hospice program. The Director of Clinical Services is responsible for ensuring that patient care is coordinated and managed appropriately. The Director of Clinical Services is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.

Responsibilities:
- Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
- Provides guidance and counseling to coordinators and Clinical Supervisors to assist them in continually improving all aspects of hospice care services, provided through organization personnel.
- Assists Clinical Supervisors in managing clinical teams and planning.
- Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
- Assists the Administrator in the preparation and administration of the organization's budget.
- Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses.
- Assures proper maintenance of clinical records in compliance with local, state and federal laws.
- Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization.
- Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures.
- Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the CHAP Home Care standards.

Qualifications:
- Registered Nurse (RN) has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state.
- Graduate of NLN accredited school of nursing with a BSN degree with 3 years of full-time clinical experience in nursing or a diploma of nursing or an associate degree in nursing and a minimum of three years of full-time or full-time equivalent clinical nursing experience in the last five years at least one year in a home health care agency or community health program which includes care of the sick at home.
- Evidence of certification by the American Nurses Association as a community health nurse or completion of at least six credits in health care management from an accredited college or university program or school of nursing.
- Previous experience in hospice/home care setting and with two (2) years of management or supervisory experience. Proven ability to work within an interdisciplinary setting.
- Complies with accepted professional standards and practice.
- Demonstrates excellent observation, verbal and written communication skills.
- Understands hospice philosophy and issues of death/dying.
- Understands principles of pain/symptom management.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
