Responsibilities:

• Participate in the development of the individualized plan of care.
• Provide exceptional Physical Therapy to patients by assessing and understanding care plan.
• Evaluate patient's functional status and home environment for hazards or barriers to more independent living. Identify equipment needs.
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
• Submits all clinical documentation, time records and other information in the required time frame.
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
• Supports company mission by providing the highest quality of home care.

Qualifications:

• Licensed Physical Therapist in the state of New York with at least 1 year of clinical experience; Home Care preferred
• COVID Vaccinated
• Proven work experience as a physical therapist
• Graduate degree or greater in physical therapy
• Valid license to practice physical therapy
• Oasis experience strongly desired
• Have valid driver’s license, reliable transportation and willing to travel 
• Some rotating weekend days required.
• Willing to work one (1) winter holiday and one (1) summer holiday per year.
• Willing to take advantage of our excellent training system comprised of in house and field orientation
• Have great communication skills, excellent organizational, and time management skills

• Ability to work independently with minimal supervision.
