Poudre Canyon is a leading provider of long-term care services, is seeking a compassionate and experienced Social Services Director to join our team. The ideal candidate will be responsible for planning, developing, organizing, implementing, and evaluating our social service programs. You will ensure that the medically related emotional and social needs of our residents are met and maintained with the highest standard of care.
This position requires an individual with a solid background in the long-term care field, with experience in Medicaid, PASSR, ULTC, and a strong understanding of the MDS process.
Key Responsibilities:
- Program Management: Plan, develop, and implement social service programs that align with the needs of the residents.
- Resident Care: Ensure the emotional, social, and medical needs of residents are met and maintained through individualized care planning.
- Compliance: Manage and stay up-to-date with Medicaid, PASSR, and ULTC regulations to ensure compliance and proper documentation.
- Care Conferences: Lead and facilitate care conferences with interdisciplinary teams, residents, and families to assess and review resident needs.
- Ancillary Coordination: Schedule and coordinate ancillary service providers (e.g., physical therapy, speech therapy, etc.) to visit and provide care for residents.
- MDS Compliance: Maintain knowledge of MDS sections relevant to social services, ensuring accurate and timely completion of assessments and documentation.
