The employee in this position, under the direction of the Public Health Manager/Health Officer provides population-based public health nursing programs and services at the individual, community and systems level. The employee in this position is expected to complete assignments with minimal level of supervision, and may lead and/or advise other public health staff in areas of programmatic expertise.  The individual in this position works collaboratively on the core public health functions of assessment, policy development and assurance. 

BACHELOR OF SCIENCE in NURSING IS REQUIRED
  • Monitors the health status of individuals, families, and the community and develops appropriate health plans and advocates for client and community need when appropriate
  • Investigates and identifies health problems and health hazards in the community, including surveillance and investigation of communicable disease reports and measures to prevent and control communicable diseases
  • Promotes the health of individuals, families, and the community through informing, educating, counseling, and appropriate intervention
  • Assesses and creates appropriate health literature and culturally appropriate health materials for both individual and community education
  • Mobilizes and leads community partnerships to address public health issues
  • Provides input into the development of policies and plans that support individual and community health efforts, and communicates and interpret such policies and plans to the public and other community agencies
  • Plans and manages a client caseload, including coordination of intra- and inter-agency services
  • Accepts responsibility for assigned program/grant management activities
  • Participates and/or in writing a in grant proposal 
  • Enforce public health laws and regulations under the authority of the Health Officer
  • Link individuals, families, and the community with needed resources 
  • Provides population-based public health nursing services in schools, homes, offices, worksites, clinics, community health fairs, and other settings in the community based on nursing and public health principles, concepts, and practices
  • Provides services aimed at the promotion of health and prevention of disease in individuals, families, and groups
  • Assists in creating a competent workforce by assisting with and participating in training, advising, and orienting public health staff and other community professionals.  
    • Participates in provision of clinical educational experiences for students in professional and allied health
    • Participates in professional development
  • Carries out public health nursing functions according to standards set forth by the Nurse Practice Acts, ANA Code of Ethics, and the Scope and Standards of Public Health Nursing Practice 
  • Participates in the evaluation of the effectiveness, accessibility, and quality of personal and population-based health services.  This includes accurate and appropriate documentation of public health interventions and gathering and review of pertinent data that support interventions
  • Conducts research for new insights and innovative solutions to health problems.        This includes:
    • Researches solutions to public health issues facing individuals, families, the community and systems.
    • Maintains an awareness of current changes and trends affecting public health and professional nursing functions.
  • Assures access to primary health care for all residents through referral or actual provision of the service
  • Fosters the understanding and promotion of social and economic conditions that support good health
  • Reviews and collects and summarizes data to interpret community health needs
  • Collaborates to adapt resources within the agency to deliver Health services based upon community need and helps in identifying financial ability of the agency
Level I: 
  • Bachelor’s Degree in Nursing from a college or university accredited by the National League of Nursing which includes preparation in Public Health Nursing

Level II includes:
  • Minimum of two (2) years’ experience in Public Health
  • Must have acceptable job performance and an additional 40 hours of applicable training in the field of Public Health
  • KNOWLEDGE, SKILLS AND ABILITIES TO PERFORM THE JOB
    • Skills required as identified by the Wisconsin Department of Health (Analytical Assessment, Policy Development/Program Planning, Communication, Cultural Competency, Community, Dimensions of Practice, Nursing and Basic Health Sciences, Financial Planning and Management, and Leadership and Systems Thinking Skills)
    • Utilizes verbal and written skills effectively and communicates clearly, intentionally and professionally
    • Ability to identify individual and community needs of specific populations including children and youth with special health care needs, pregnant women, persons affected with chronic disease or environmental hazards and their families and seeks out or provides interventions to meet these needs
    • Ability to read and interpret documents, and laws in the English language
    • Ability to listen to and understand information and ideas in spoken and written format so individuals may effectively understand and convey communications
    • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
    • Ability to compute rate, ratio, and percent
    • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form
    • Inductive and deductive reasoning – ability to combine separate pieces of information, or specific answers to problems, to form general rules or conclusions; includes coming up with a logical explanation for why a series of seemingly unrelated events occur together and deciding if an answer makes sense
