Job Responsibilities
The Kent County Health Department seeks a qualified Outreach Worker to provide outreach to eligible individuals to promote the utilization of services offered through the WISEWOMAN Program. The individual will serve as a liaison between the lay navigator with the Michigan Department of Health and Human Services and health care providers and clients in the county network. Responsibilities include providing information to clients concerning program eligibility for services, assisting clients with applications, eligibility documentation, billing questions, oversight, and all other matters related to program participation. Works closely with current client providers and billers to keep them knowledgeable of program requirements as well as recruit and onboard new providers.
Ability to speak Spanish is beneficial.

Minimum Qualifications
This position requires a high school diploma or GED.
Additional requirements include the ability to interact positively with clients from a wide range of cultural and socio-economic backgrounds and with widely divergent levels of educational attainment and social skills. Experience working in a position requiring significant public contact is desired. Post high-school class work and/or Community Health Worker certification would be beneficial.
